The mega-corporate Daedalus Group is under attack by a small group of activists known as the Ghost of Icarus. While taking care of some of the victims of the bombing, Black Jack encounters Dr. Kiriko, also known as Death Incarnate, who just like him, asks his patients huge sums of money but instead of saving them, he offers them a painless death. When the activist group requests both doctors’ collaboration, Black Jack finds himself caught in a spiral of life, death and treason while trying to save his patients' lives and avoid the breaking of a biological war that could destroy humanity.

The TARDIS arrives in 1925 England where the Doctor ends up playing in a local cricket match due to a case of mistaken identity. The travellers accept an invitation to a masked fancy dress ball, but events take on a more sinister tone as murders are perpetrated at the country home of their host, Lord Charles Cranleigh.

April 1, 1699: Captain Henry Avery writes in his journal onboard the Fancy. They have been stranded for eight days for lack of wind. All they can do is wait. He writes of an enemy who comes from the still ocean and takes members of the fearful crew. The captain feels an evil presence watching him and longs for the wind to pick up so they may leave, but he feels they are all doomed to die.

A series of suspicious death cases take place. All the victims were terminal patients. Detective Hayato Inukai (Gou Ayano) and Detective Asuka Takachiho (Keiko Kitagawa) investigate the case. Through their investigation, they learn of a person known as "Dr. Death." He performs euthanasia by request. Meanwhile, Detective Hayato Inukai’s daughter Sayaka requests the help of Dr. Death. Sayaka suffers from a severe kidney disease.
