Dorothy is a university student living in a bustling city. She serves as the concertmaster of her school’s orchestra and is also an accomplished professional violinist outside of school. As she decides to schedule an extra rehearsal ahead of her final concert before graduation, tensions rise among orchestra members, each struggling with their own lives. Having always been relied upon, Dorothy is shaken by the backlash. Overwhelmed, she runs out of the rehearsal room and vows to give up the violin. Biohazard ~ Voice of Gaia ~ is set in an unspecified year where the threat of bioterrorism has continued to increase in extremity. Much of the story is set in and around the walled Croatian city of Dubrovnik, on the Adriatic seacoast, which faces attack from "Them", zombies infected by an ancient virus. The survivors in Dubrovnik are protected by both their wall and by music, as melodies are known to calm Them, suppressing their violent urges.

Ayumi Aizawa is a lonely young man without a family. He works at Forest Hyde Press Hotel as a cleaner. His deceased grandfather Kenzaburo worked at the same hotel. When his grandfather worked at the Forest Hyde Press Hotel, the hotel was well known for its extreme hospitality. Now, the hotel is nothing more than an ordinary hotel.
